Appointments
To schedule an appointment for our health services, please call 323-653-1990, Monday through Thursday from 7:30 a.m. to 4:30 p.m, Friday 7:30 a.m. to 4:00 p.m.
To cancel an appointment, call the same phone number. Please cancel as soon as you know that you will not be able to make an appointment and no later than 24 hours before your scheduled appointment. A patient who missed three appointments will not be allowed to make an appointment without review of their record.
Medi-Cal, HMO, and Underage Patients

What to Bring to Your Appointment
Due to funding requirements and other health care regulations, you will need to bring the following to your next appointment: Proof of Income (paystub, unemployment check, letter from Social Security, tax return), Proof of Address in Los Angeles County (utility bill, lease agreement, California driver's license or ID), and Proof of Citizenship (birth certificate, green card, passport, certificate of naturalization). You will not be denied services based on your income or citizenship status.

Please also bring your appointment card and BIC medical card.
In order to protect our patients' identities, photo IDs are required before services can be provided. Acceptable forms of photo IDs include a California Driver's License, California Identification Card, an ID from your country of origin, the Marticular Consular ID from Mexico, a passport and school ID.

Health Coverage Programs
The Saban Free Clinic can help determine whether you are eligible for a variety of free health coverage programs. One such program is Healthy Way L.A. -- a Los Angeles County NO COST coverage program that gives you high quality medical care with more flexibility and coordination. Through Healthy Way L.A. you can select The Saban Free Clinic to be your medical home so that we become the one place you go for all your medical needs.
Healthy Way L.A. provides benefits to cover all your medical costs and offers you incentives such as a convenient 24/7 nurse hotline, access to medical specialists and in-house pharmacies, the opportunity to schedule priority appointments at the Clinic, emergency medical coverage, and so much more.
To qualify for Healthy Way L.A. you must have proof of the following:

Los Angeles County resident

21*-64 years of age (under 21 may be eligible if denied by Medi-Cal)

U.S. citizen/legal permanent resident five years or more

Monthly income no more than $1,207 for a family of one

Not pregnant or eligible for Medi-Cal or Healthy Families
You must bring in the following required documents to determine your eligibility:

Proof of Income: Can be a salary paystub, an unemployment check, an award letter from Social Security, or an income tax return.

Proof of Address: Can be a utility bill with your name on it, a letter from the person you are living with, a California identification card, or a California Driver's License to show you live in Los Angeles County.

Proof of Citizenship: Can be a birth certificate, a green card, a passport, or a certificate of naturalization.
If you are not a U.S. citizen, you are still encouraged to bring in proof of income and address verification as you may be eligible for a variety of other no-cost or low-cost health care programs.
These documents are used solely for determining your eligibility for health care programs and will NOT be submitted to the Immigration and Naturalization Service (INS) or Internal Revenue Service (IRS).
